  • The application form below, with all required fields completed.  If you prefer, you can also call 503-638-1536, 8 am - 4pm PST, M-F and submit your application over the phone.
  • A copy of both your business license and resale license, attached below or mailed to ORNAMENTS TO REMEMBER, P.O. Box 3440, Wilsonville, OR 97070-3440.
  • Once your application is received, we will contact you for additional credit information.

Once your application is approved, we will contact you using the method you prefer, and supply you with a Username and Password, as well as instructions for logging in.
